For example, you may prefer to have an identification wrist band to put on your children or parents when you travel, or you may wish to add more storage space to your archive to accommodate large files such as MRIs and x-rays.
For convenience, you may choose to have us send your medical history to you on a CD that you may then take with you to physician appointments or when you travel.
Preventive Health and Health Insight
MyHealthArchive not only helps you get information to your physician and hospital, it also helps you keep track of your health.
Even more, it can remind you when you need to take medicine, renew your prescription, arrange an appointment, and more.
You could come home from a doctor visit and set up email reminders to yourself that arrive a month later to go get your mammogram or a year later to call for a follow up appointment.
A major breakthrough in personal medical information is MyHealthHistory, a comprehensive step by step process to help you recall and record your medical history.
When you give your medical history to your doctor he or she writes a summary of your history, but often you don't see what is written. If you could write down your history, you might be prompted to remember additional events.
If you saw what the physician writes, you could be sure that it is accurate and inclusive. MyHealthHistory promotes both getting a full, inclusive history and eliminates discrepancies, or unintentional deletions.
Some physicians ask you to complete a short questionnaire, but MyHealthHistory is far more complete than the usual questionnaire, and over time as you update your archive, you will have the invaluable record of your lifelong medical history. MyHealthHistory is at your fingertips when you need it.
Show your archive to your physician to improve the information base he or she uses to make decisions about diagnoses, laboratory and imaging tests, and treatments. Our technology eliminates paper and pencil lists, and the errors and losses they may cause.
Employ USERNAMEs and PASSWORDs to Control How and to Whom Your Information is Released
With your subscription you will receive a USERNAME and PASSWORD which you can change as you desire. Don't share your USERNAME and PASSWORD with anyone who you don't trust with your medical history.
You may choose a Family USERNAME and Family PASSWORD so family members can access the information if you are incapacitated and cannot access your information. You may choose to release some, but not all, information to someone who has your Family USERNAME and Family PASSWORD.
You may also choose an Emergency USERNAME and Emergency PASSWORD that can be used by your physician or emergency medical technician to get the information you have predetermined to release in case of emergency or disaster.
You may change your USERNAMEs and PASSWORDs. as often as you desire. For example, you may choose to give your Emergency USERNAME and Emergency PASSWORD to a physician you see one time, and if you felt you wanted to stop access by that physician after the visit, you could change the Emergency USERNAME and Emergency PASSWORD later to block further access.
Once you have a subscription to MyHealthArchive you can take your time to collect all the medical information you can locate. You can take your time to enter your medical history to record your recollection of the events.
Some people will want to put in enormous detail. Others will prefer a summary of details. Suit yourself. This is your health care Archive.
