How does the Concierge Service Work?
Step One: Obtaining your medical records:

1. In order to take advantage of MyHealthArchive Concierge Service, you or the person for whom you are arranging the service must be a member of MyHealthArchive.

2. To begin the process we will ask you to complete an Online Concierge Service Order Form which provides detailed information as to how a MyHealthArchive Concierge can reach you.

3. The Initial Service starts with a 30 minute consultation with a MyHealthArchive Concierge who will organize the information gathering process with you. When the MyHealthArchive Concierge calls you, you will be asked to provide the password for the account. If you do not have the account password, the MyHealthArchive Concierge will not be able to access any of the information in the account, including not being able to add any information to the account. The Initial Service includes creating a Plan of Action specific to your situation. The Plan of Action will set forth the health care providers who will be contacted on your behalf, and the steps to be taken to assist you with the medical records you receive. We’ll discuss which of your specific records should be ordered to provide the level of detail you desire to have in your Archive. Our fee for this Initial Service is $120.00, payable in advance to your ‘Account’ section.

4. You can download a release of medical information form (often called a medical authorization form) or have one emailed or FAXed to you. The medical authorization form must be completed by you in order to authorize health care providers to release your medical records to MyHealthArchive. It is very important that you tell us how you want your records handled:

            A) Have them sent to your address so you can upload them into your Archive yourself, or

            B) Have them sent to MyHealthArchive so we can upload them for you.

Once your medical authorization form is signed, we will begin the process of contacting the health care providers that you have authorized us to contact in your Plan of Action.  Some health care providers may provide records free but most will charge a fee. Your MyHealthArchive Concierge will inform you of fees they are told you will need to pay to healthcare providers to have your records released. We encourage healthcare providers to provide the medical records electronically which markedly reduces or eliminates fees to be paid to healthcare providers for medical records.

5. Once we receive payment from you, we will request copies of your medical records from the specific sources that you have authorized.  We cannot complete the process of requesting your medical records until we receive the requested fees to send to your health care provider, because they generally will not release the records without payment.

6. We’ll notify you when we receive all requested copies of your medical records included in your Plan of Action. If you have asked us to upload your records we will inform you when that has been completed. If we identify problems or delays obtaining your medical records we will notify you, as well.

Step Two: Uploading your medical records into your Archive:

If you or a loved one needs help uploading your medical records, MyHealthArchive Concierges are happy to assist. Our fee for Uploading Service is $60.00 for the first hour, then $25.00 per half hour or part thereof, payable in advance to your ‘Account’ section. You will need to keep funds in your account to cover additional expenses as they occur.

Step Three: Compiling and organizing your health history in your Archive:

If you or a loved one needs help compiling a complex health history in your Archive, your MyHealthArchive Concierge is trained to assist you or a loved one with this process. Our fee for Compiling service is $60.00 for the first hour then $25.00 per half hour or part thereof, payable in advance to your ‘Account’ section of your Archive. You will need to keep funds in your account to cover additional expenses as they occur.

Step Four: Ongoing Assistance in maintaining your Archive:

If you need us to continue to assist you in obtaining additional records, updating records as new events occur (such as a new physician visit), you may want to sign up for Ongoing Assistance. We will update your Plan of Action so that you and your MyHealthArchive Concierge have a clear understanding of what you are asking us to do for you. Your MyHealthArchive Concierge will then assist you long term with maintaining your health information in your Archive, as agreed. Our fee for Ongoing Assistance is $60.00 for the first hour then $25.00 per half hour or part thereof, payable in advance to your ‘Account’ section of your Archive. You will need to keep funds in your account to cover additional expenses as they occur.
